- I can verify that I am at least 18 years of age. - I understand that I can only book an appointment for myself. - I understand that there is a non- refundable $50 fee to reserve my spot in line for the desired day due immediately upon receipt. - I understand that if I want more than one design that an additional $50 feet is required for each tattoo. - I understand that this fee is not part of my tattoo total. - I understand that I can not schedule a specific time, but will be booked first come first served & assigned a spot in line for my desired day once I have paid my booking fee. - I understand that if I pay booking fees for multiple tattoos but don’t get them all done on the day of the event, that I will lose the booking fees for those designs. - I understand that the flash options are final in size, design, and price, and are limited to arm and leg placement only. - I understand that the cost of each tattoo is $250+tax - I understand that I can pay cash or card, but card will incur a 3% cc fee. - I understand that my artist may refuse my service if I am intoxicated or for any other reason.
